Job summary

Delta Health Center, Inc. is seeking a Community Health Worker (CHW) to support opioid education, naloxone distribution, and community outreach across its six-county service area.

The CHW will plan, coordinate, and conduct outreach at clinics and in communities, building relationships with patients, families, schools, faith-based organizations, and local partners to raise awareness and facilitate access to prevention and treatment resources.

Qualifications

• High school diploma or equivalent.
• Experience working with patients, families, community members, or vulnerable populations.
• Strong communication and interpersonal skills.
• Ability to conduct public presentations and small-group educational sessions.
• Ability to travel throughout DHC's six-county service area.
• Ability to maintain accurate documentation and meet grant reporting deadlines.
• Basic computer skills, including Microsoft Office applications.
• Ability to maintain confidentiality and comply with HIPAA requirements.

Responsibilities

• Plan and conduct public outreach events.
• Distribute fentanyl test strips and provide education.
• Provide one-on-one and small-group OEND sessions.
• Maintain records and grant reporting.
• Coordinate referrals to healthcare and social services.
• Build relationships with community partners.
