Qualifications

• High School Diploma required; higher degree preferred.
• Valid driver’s license and reliable transportation required.
• 1+ years in community outreach, health education or related field strongly preferred.

Responsibilities

• Conduct community outreach and engage community members to understand their health needs.
• Provide individual and group health education and coaching to community members.
• Assist community members in accessing and navigating the healthcare system, including referrals to health services.
• Provide basic health screenings and assessments to identify health risks and interventions.
• Assist in developing health promotion programs and activities addressing community needs.
• Collect data and maintain records of outreach activities and interventions.
• Attend meetings and trainings related to community health promotion and education.
