Job description

Overview

Provides education, member advocacy, and accompaniment to medical care appointments as determined by team leaders. Participates in team meetings to discuss regular evaluation and feedback about clients on caseload. Works with clients to determine their eligibility for social service programs and identify barriers to medical care and follow-up. Alerts team members when supportive social and community services are needed and ensures access and linkage to edu and health plan services. Contributes to client engagement in medical care as prescribed by their medical team. Works under close supervision.

What You Will Do
  • Review rosters with supervisor to identify members needing follow-up and assistance
  • Notify and assist with immediate needs and risk factors; refer to services accordingly
  • Coach members using motivational interviewing to address medical and psycho-social health; support participation in care plans, including home visits and appointments
  • Conduct outreach via phone and face-to-face for follow-up on self-care, medication, scheduled visits, and test results
  • Reinforce provider education on disease management; educate clients about conditions and importance of follow-up
  • Collaborate with outreach team to discuss progress, barriers, and health equity issues; participate in designing follow-up strategies
  • Assist clients with navigating the health system, including accompaniment and advocacy for access and appointments; prepare detailed case notes
  • Engage hard-to-reach members with overdue screenings or missed appointments
  • Assist clients in obtaining and attending medical appointments, including accompaniment as needed
  • Use motivational interviewing to identify barriers to engagement in care
  • Provide guidance on self-management tools and health literacy
  • Facilitate case reviews and conferences with providers
Qualifications

Licenses and Certifications: Peer navigation or CHOW training preferred

Education: High School Diploma or equivalent required; Associate's in human services or related field preferred

Work Experience: Minimum one year in care management, community health, social services, or medical practice preferred; experience with underserved communities, lived experience, and working with mental health/substance use populations preferred; bilingual skills and basic computer skills required; empathetic communication style necessary
