Job summary

• High School Diploma or equivalent is required.
• CHW Certification preferred; required within 12 months of hire.
• Valid driver's license with good driving record is required.
• 1 year of recent experience in a community outreach or healthcare setting preferred.

Responsibilities

• Builds trusting relationships with individuals and communities to mediate between individuals and health/social service systems.
• Assesses needs and connects individuals to appropriate resources, including housing, food, and transportation.
• May conduct outreach activities, including home visits and community events, to engage underserved populations.
• Maintains accurate records to support advocacy efforts and program evaluation.
• Supports ongoing community health programs and assists with health education and basic screenings such as blood pressure and glucose checks.
