Baptist Medical Center in Downtown Jacksonville is seeking a Community Health Worker to join its care coordination team. This full-time, day-shift role focuses on improving patient outcomes and strengthening community health.

The CHW will bridge healthcare providers with community services, build trust within diverse communities, support a primary care medical home model, and educate patients on managing chronic conditions to reduce disparities.

Qualifications

• 1–2 Healthcare experience required
• High School Diploma required
• Driver's License - DMV required
• Certified Community Health Worker - varies by program

Responsabilités

• Serve as a bridge between healthcare providers, community service organizations, and patients to improve care coordination and access.
• Build trusted relationships within the community by understanding patients’ cultural, social, economic, and language needs.
• Support patients within a primary care medical home model, ensuring continuity of care.
• Provide health education and guidance for managing chronic conditions such as Diabetes, Cardiovascular disease, and COPD.
• Help reduce health disparities and improve overall quality of life for the populations served.
