Job summary

Siena Hospital, part of the CommonSpirit Health network, is hiring a Community Health Worker to bridge clients with health and social resources in the Henderson area. You will assess needs, connect to services, and provide coaching to improve access and outcomes.

Qualifications

• High School Diploma or GED and experience working in the community is required.
• Bachelor's degree is preferred and Teaching experience is a bonus.
• Community Health Worker Certification is preferred to enhance qualifications.

Responsibilities

• Advises clients or community groups on diabetes, obesity, health, diet, exercise, and related topics.
• Attends community meetings and health fairs to understand issues and build relationships.
• Advocates for health needs with government agencies or health service providers.
• Identifies high-risk or targeted groups and provides cultural mediation for health services.
• Provides one-to-one coaching to individuals seeking this service.
• Refers community members to needed health services.

As our Community Health Worker, you will be a vital link between individuals and essential community resources, actively working to reduce health disparities and improve access to support services.

Every day, you will proactively engage with clients, assessing their needs and identifying pathways to care and support. You'll connect individuals to relevant health, social, educational, and employment resources, providing guidance and practical assistance to overcome logistical barriers.

To be successful in this role, you will possess a profound understanding of diverse community needs, exceptional problem-solving abilities, empathy, and resilience. Your dedication to empowering clients and removing barriers to care will be paramount.

• Advises clients or community groups on issues related to diabetes and obesity, improving general health, diet and exercise, prevention, sanitation or hygiene, self care, social issues, education, childcare or problem solving.
• Acts as a resource to all clients, providers and the community. Attends community meetings and health fairs to understand community issues and build relationships with community members. Distributes flyers, brochures, or other informational or educational documents to inform members of a targeted community. Refers community members to needed health services.
