Job summary

NovumHealth in Las Vegas, NV is hiring a Community Health Worker to support individuals navigating mental health and substance use services. You will engage with clients in the community or at home, coordinate care, and connect them with resources to promote wellness.

Qualifications

  • Minimum 1 year of experience in community health or related roles.
  • Active Community Health Worker Certification required.
  • Willingness to obtain CPR/First Aid if not already held.
  • Ability to travel locally and provide transportation for members when needed (mileage reimbursed).
  • Must pass drug screening and background check.

Responsibilities

  • Assessing healthcare needs and identifying gaps in care.
  • Collaborating with healthcare providers and community organizations to develop care plans.
  • Assisting patients in navigating the healthcare system and accessing services.
  • Coordinating appointments for patients with multiple providers.
  • Facilitating communication between patients, providers, and partners.
  • Monitoring progress and supporting adherence to care plans.
  • Conducting home visits to assess living conditions and support systems.

Since 2015, Nevada Behavioral Health Systems (NBH) has been providing mental health and substance abuse services to residents across Nevada. By collaborating with a diverse network of experienced behavioral healthcare providers, we have built a community-based care system that offers members personalized options tailored to their individual needs. In 2023, we rebranded as NovumHealth with the expansion of our services in Arizona, California, and Utah.

Position: Community Health Worker

Location: Las Vegas, Nevada – Onsite (This role is not eligible for Remote or Hybrid)

Starting Salary Range: $23 per hour

The starting salary range for this role is a reasonable estimate, with a goal to pay a competitive market salary focusing near the median of our pay ranges. However, final offers for all positions will be based on several factors such as experience level, education and certification, skills, and internal pay equity.

What's the role?

Community Health Workers (CHWs) at NovumHealth are trusted members of the community who help individuals navigate challenges with mental health, substance use, and social barriers. CHWs meet people where they are, whether at home, in the community, or in recovery settings. Building trust, providing support, and connecting members with care and resources.

If you have experience in behavioral health, peer support, or recovery communities, this role gives you the opportunity to make a direct impact by helping individuals access the services they need and overcome obstacles on their journey to wellness.

What you'll do:
  • Assessing the healthcare needs of individuals within the community and identifying gaps in care.
  • Collaborating with healthcare providers, social service agencies, and community organizations to develop comprehensive care plans for patients.
  • Assisting patients in navigating the healthcare system and accessing necessary services, such as medical appointments, specialist referrals, diagnostic tests, and community resources.
  • Coordinating and scheduling appointments for patients with multiple healthcare providers to ensure continuity of care and optimal care coordination.
  • Facilitating communication between patients, providers, and community partners.
  • Monitoring patients' progress and adherence to care plans, providing support and education to promote selfmanagement and prevention.
  • Conducting home visits or outreach activities to assess patients' living conditions, social support systems, and adherence to treatment plans.
  • Advocating for patients' needs and rights within the healthcare system and community.
  • Documenting patient interactions, care plans, and progress notes in electronic health record and care management systems.
  • Participating in interdisciplinary team meetings and case conferences to ensure effective coordination and collaboration with other healthcare professionals.
  • Provide transportation, with personal vehicle, for members to appointments when needed (mileage is reimbursed).
  • Additional duties as assigned.
  • Building strong relationships with individuals and families to promote trust and reduce stigma.
What you'll bring:
  • Comprehensive knowledge of healthcare systems, community resources, and social services available to support patient care.
  • Knowledge of behavioral health, substance use treatment, and community resources.
  • Strong interpersonal and communication skills to establish rapport with patients, families, and healthcare providers.
  • Excellent organizational and time management skills to manage multiple patients and coordinate care effectively.
  • Ability to assess patient needs, develop individualized care plans, and evaluate outcomes.
  • Proficiency in using electronic health records and care management software.
  • Understanding of privacy regulations and compliance requirements related to patient information and data security.
  • Compassionate and patientcentered approach to care, with a commitment to advocating for vulnerable populations.
  • Problemsolving and critical thinking skills to address barriers to care and resolve complex situations.
  • Cultural competence and sensitivity to diverse populations and their unique healthcare needs.
  • Ability to work collaboratively in a team and maintain positive relationships with healthcare professionals and community partners.
What you'll need to start:
  • At least 1 year of experience in community health, behavioral health, peer support, or related roles.
  • High school diploma or equivalent required.
  • Bachelor's degree in healthcare, social services, or related field preferred.
  • An active and valid Community Health Worker Certification.
  • CPR/First Aid (Or willingness to obtain)
  • Must pass drug screening and background check in order to start employment with NovumHealth.
  • Must have independent transportation vehicle to aid in transporting members (mileage reimbursed).
  • Must be willing to receive vaccinations in accordance with hospital and state requirements.
