Location: Brooklyn NY
(We have multiple Community Habilitation Worker positions available acrossvarious locations in Brooklyn,each with a different schedule. Availability includes weekdays, evenings, and weekends. Let us know your preferred location and schedule when applying!
Pay Rate: $19.00/hr.
Hours: 3-20 per week


Community Habilitation Worker

Key Responsibilities
  • Provide person-centered support in homes and the community.
  • Assist individuals in building life skills, relationships, and self-advocacy.
  • Ensure safety, well-being, and adherence to individual care plans.
  • Encourage community involvement and inclusion.
  • Maintain accurate documentation and uphold compliance standards.
  • Communicate effectively with individuals, families, and team members.
  • Participate in training and professional development.
Qualifications
  • High School Diploma or GED.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic computer skills for documentation and communication.
  • Valid driver’s license preferred.
  • Bilingual in English & Bengali is a plus!
Work Environment & Physical Demands
  • Community-based and home settings.
  • Ability to perform physical tasks such as standing, lifting, and walking.
