Job summary

SCO Family of Services seeks a Community Habilitation Specialist to assess, train, and assist developmentally disabled individuals enrolled in HCBS Waiver programs. You will work in the consumer’s home or community, following the Community Habilitation Plan and ISP-driven hours.

The role emphasizes promoting independence, personal growth, productivity, and community inclusion while maintaining regular communication with supervisors and families. A valid NYS driver’s license is required.

Qualifications

• High School Diploma (minimum) and proof of Valid NYS driver’s license REQUIRED.
• Experience communicating with individuals and families in a caregiving or supportive capacity preferred.

Responsibilities

• Provide assessment, training and assistance to developmentally disabled individuals enrolled in the HCBS Waiver program.
• Support consumers in their homes or community based on the Community Habilitation Plan outcomes.
• Coordinate with Supervisor(s) about consumer needs, time off, and performance; maintain monthly driver authorization and related issues.
• Communicate changes in schedules, lateness, absences and other issues that affect service delivery.
• Ensure ongoing focus on independence, individualization, productivity, and community inclusion.

Job description

• The Community Habilitation Specialist will provide assessment, training and assistance to developmentally disabled individuals enrolled in the HCBS Waiver program and living in a home situation.
• The Community Habilitation Specialist will work with the consumers in their homes or community, as needed, based on the valued outcomes listed on their Community Habilitation Plan.
• The specific amount of hours to be worked on, is predetermined by the consumer’s Individualized Service Plan (ISP).
• The focus of this position is to facilitate an individual’s independence, individualization, productivity, and inclusion in their community.
• The Community Habilitation Specialist must maintain communication with their Supervisor(s) about their consumer(s), time off needed, job performance and must be an SCO Family of Services authorized driver and authorized to drive and other relevant issues at least once a month via telephone.
• The Community Habilitation Specialist is expected to communicate with their Supervisor(s) and consumer(s)/and family about changes in schedules, lateness, absences, and other issues that will affect working with the consumer. In addition, the following areas are required to maintain a position as a Community Habilitation Specialist.
• High School Diploma (minimum) and proof of Valid NYS driver’s license REQUIRED.
