Job summary

LIFESPAN is hiring compassionate Community Guides to help participants and planning teams develop social networks in their local communities. This full-time role is based in Center City—Charlotte, NC, with a day shift and flexible hours to support ISP goals.

Qualifications

  • Must be at least 18 years old.
  • Valid driver’s license for a minimum of two consecutive years.
  • Pass criminal record and healthcare registry check.
  • High school graduate or equivalent (GED).
  • Commitment to achieve competency in required knowledge, skills, and abilities.
  • First Aid/CPR certification required; training provided if needed.
  • Ability to lift up to 40 pounds independently.

Responsibilities

  • Inform and coordinate community resources among primary, preventative, and chronic care providers.
  • Assist in locating and accessing non-Medicaid community supports related to ISP goals.
  • Assist in locating housing options and furnishing the personal residence.
  • Provide instruction and counseling guiding problem solving and decision making.
  • Advocate on behalf of the participant.
  • Support the participant in preparing and implementing the ISP.
  • Coordinate services with the Agency with Choice or self-direction budgets as needed.
  • Be available to work flexible hours including nights, weekends and overtime.
  • Assist with transportation and mobility needs as required.

Monday through Friday 8:00 AM to 4:00 PM/ 9:00 AM to 5:00 PM

What you’ll do


  • Responsible for Informing and coordinating community resources including coordination among primary, preventative, and chronic care providers.

  • Assist in locating and accessing non-Medicaid community supports and resources that are related to achieving Individual Support Plans (ISP) goals.

  • Assist in locating options for renting or purchasing a personal residence, assisting with purchasing furnishings for the personal residence.

  • Provide instruction and counseling which guides the participant in problem solving and decision making.

  • Advocate on behalf of the participant.

  • Support the person in preparing, participating in and implementing the Individual Support Plan (ISP).

  • Provide training on the Individual and Family Directed Supports Option, if the participant is considering directing services and supports.

  • Assist management with guidance of the individual budget.

  • Assist with coordinating of services with the Financial Supports Services provider, if the participant is self-directing services under the Employer of Record Model, including guidance on use of the individual and family directed budget (self-directed budget).

  • Provide information on recruiting, hiring, managing, training, evaluating, and changing support staff, if the participant is self-directing services.

  • Assist with the development of schedules and outlining staff duties, if the participant is self-directing services.

  • Assist with understanding staff qualifications, documentation and record keeping requirements, if the participant is self-directing services.

  • Provide on-going information to assure that participants and their families/representatives understand the responsibilities involved with self-direction, including reporting on expenditures and other relevant information and training.

  • Coordinate services with the Agency with Choice if the participant is directing services under the Agency with Choice Model.

  • Be available to work flexible hours including nights, weekends and overtime as required.

  • Depending on the person you support, you may be required to:

    • Administer medications. Training is provided. Nursing team supports you.

    • Provide transportation to and from designated sites.

    • Assist people with mobility needs, e.g. utilize transfer techniques, a Hoyer lift and/or gait belt. Training is provided.





Qualifications


  • Must be at least 18 years of age

  • Possess a valid driver’s license for a minimum of two consecutive years

  • Pass criminal record and healthcare registry check

  • Pass driving record check if providing transportation

  • High school graduate or equivalent (GED)

  • Commitment to achieve competency in required knowledge, skills, and abilities

  • Note: Current First Aid/CPR blended or classroom certification is required. If you do not currently have certification from an approved provider, our American Red Cross certified instructors will provide you the training.

  • Ability to lift up to 40 pounds independently (per policy)
