Qualifications

• Master's degree in social work from an accredited school.

• Experience in health care settings with pediatric through geriatric patients preferred.

Responsibilities

• Provide comprehensive social work services to help patients and families overcome barriers to health.
• Conduct psychosocial assessments and coordinate referrals and education for prenatal patients.
• Develop and implement interventions and treatment plans with interdisciplinary teams.
• Maintain accurate records reflecting outcomes and compliance with regulations.
• Educate clients, families, and professionals on disease prevention and health maintenance.
