Job summary

Sonoma Valley Community Health Center (SVCHC) is seeking a full-time, on-site Family Medicine Physician to provide comprehensive primary care to individuals and families in Sonoma Valley. You will conduct patient assessments, diagnose and manage acute and chronic conditions, and deliver preventive care including screenings and immunizations.

You will work within an interdisciplinary team, coordinate referrals, document in EPIC, follow evidence-based guidelines, and participate in quality

Qualifications

• Valid, unrestricted California medical license.
• DEA registration to prescribe medications.
• Board certified or board eligible in Family Medicine.
• Medical Spanish preferred.
• Experience documenting in EHR systems (EPIC).

Responsibilities

• Provide comprehensive outpatient family medicine across the lifespan.
• Manage acute and chronic illnesses and preventive care.
• Coordinate referrals to internal and external services.
• Document care accurately in EPIC and maintain records.
• Collaborate with interdisciplinary care team to address medical, behavioral, and social determinants of health.
• Participate in CQI activities and professional development.
