Job summary

The CARE Center, Inc. in Butler, PA is seeking a Part-Time Social Services Specialist to implement and monitor client service plans and assist in daily program operations.

You will conduct psychosocial assessments, provide supportive counseling, and advocate for entitlement resources while maintaining client records and coordinating with community partners. Qualifications include a Bachelor’s degree in Social Work or related field and successful clearance checks.

Qualifications

• Bachelor’s degree in Social Work or related human services field (accredited).
• Ability to obtain vital information during interviews to inform recommendations.
• Knowledge of financial entitlement programs and available social services.
• Must obtain Criminal History Clearance from PA State Police, DHS, and FBI.
• Must not be excluded from Medicare/Medicaid or federal health programs.

Responsibilities

• Implements and monitors client service plans and assists in daily operation of program activities.
• Conducts psychosocial assessments and reassessments to develop and update service plans.
• Provides supportive counseling and intensive life skills training to individuals and groups.
• Advocates to identify entitlement resources and social/vocational services.
• Maintains client records, prepares reports, and ensures timely documentation.
• Maintains communication with internal and external programs and community resources, adhering to policies and laws.
