The role emphasizes individualized service plans, safety, and community integration to promote independence and social engagement. Responsibilities include documenting progress, promoting healthy routines, and executing behavior plans while supporting individuals to remain work-ready and safe.

Qualifications

• High School Diploma, GED or equivalent is required.
• Valid Driver's License and acceptable driving record required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Facilitate the development of valued community roles and personal relationships.
• Review and complete all required documentation.
• Ensure the health, safety, and well-being of all individuals.
