Job summary

The Christ Hospital Home Health Care is seeking a Community Educator/Account Executive to inform healthcare professionals about our home health services and to represent the company in the community. You will build relationships with physicians, hospitals, and long-term care facilities while driving new referrals and improving patient transitions of care.

Responsibilities

  • Educate healthcare professionals about the Company services (physician offices, long-term care facilities, rehabs and hospitals).
  • Develop and maintain relationships with healthcare professionals in surrounding communities.
  • Create awareness in the community of the need and benefits of home healthcare services.
  • Network and attend business organizations on behalf of the Company.
  • Educate clients and prepare them for services; coordinate new referral admission with internal staff.
  • Follow-up with referrals to assess service progress via calls, letters and visits.
  • Prepare weekly marketing reports for supervisor and monthly expense reports.
  • Set weekly, monthly and yearly goals for new accounts, referrals and customers.
  • Attend trainings and agency meetings and stay updated on Medicare/Medicaid/Home Health policies.

The CE professionally represents the Company to attain and maintain accounts throughout the community. May attend discharge/multidisciplinary rounds in acute care, ambulatory and or other settings within the health system to share expertise and to assist the patient in transition of care from one setting to the next within the health system.
