TheCommunity Diversion Coordinator is responsible for providing services andsupports to individuals living with mental illness who are involved or at riskof being involved with the criminal justice system. This includes individualswho are participating in the Mental Health Court Program or otherwise mandatedto participate in mental health services. The Community Diversion Coordinatorwill assess the individual's strengths and needs and, in collaboration with theindividual, develop a person-centered recovery plan. The Community DiversionCoordinator will provide skills training, supported housing, supportedemployment, psychosocial rehabilitation, assessment, linkage, and referral tosupport goal progress for individuals and families living with mental healthchallenges. The Community Diversion Coordinator is responsible for schedulingand carrying out individual and group services. This position will be requiredto be trained and certified in crisis intervention. Office hours are M-F 8:00am to 5:00 pm, and extended hours one day a week; however, the position musthave flexible hours to accommodate individuals who cannot meet between 8:00 amand 5:00 pm. This position will require travel, home visits, and serviceprovision in community settings. This job is community-based and requires theability to work autonomously with little supervision.
Youwill get to:
- Attends CourtStaffing meetings and Court hearings.
- Works closelywith court staff to ensure continuity of care.
- Ensures that allindividuals on caseload have a current Uniform Assessment (at least 95% of allCANS / ANSA's are completed on time).
- Achieves anaverage score of 85, with no one score below 75 on the Person CenteredRecovery Plan, Quality Management audits.
- Completes initialinterviews and enrollments for individuals new to services under the guidanceand supervision of an LPHA.
- Provides casemanagement services to justice-involved individuals assigned to a caseload.
- Schedules anddelivers services that meet performance targets relevant to the position.
- Responsible for ensuring staff kept appointment rates of at least 80%.
- Must providecollaborative documentation at least 95% of the time or no later than 24 hoursafter delivery of service.
- Provides averagehours as required by the Level of Care served.
- Will deliverservices using the recommended Evidence-Based Practices recommended by HHSC.
- Person CenteredRecovery Planning is developed in collaboration with the individual/familyserved and any other person or provider requested by the individual/family.
- Responsible for the provision of crisis intervention services as needed.
- Must be crisiscertified within 120 days of employment.
- Attends workregularly and as scheduled.
- Workscooperatively with others and the supervisor
- Communicateseffectively and in a timely manner with team and individuals served.
- Follow the agency'sworkplace values and principles.
- Maintain clientconfidentiality and privacy according to standards.
- Follow policy andprocedures for protecting clients as well as reporting client abuse and neglectif needed.
- Protect andensure client rights always.
You'lldefinitely need:
- Bachelor'sdegree from an accredited college or university in social, behavioral, or humanservices, and the ability to be credentialed as a Qualified Mental Health Professional (QMHP-CS).
- Maintain a valid Texas state Driver's License and auto insurance
- Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter.
- Preferred: 3-5 years of experience working in a mentalhealth setting providing community-based services within the community.
