Job Summary:

The Community Development Services (CDS)/Employment Supervisor will oversee most facets of care and treatment necessary to provide quality Community Development services and is responsible for the operations and staff supervision of the CDS program, with additional areas of responsibility as delegated. The CDS/Employment Supervisor is a lead care level position at EBED Community Improvement, Inc. and may be required to work as direct care if needed. Additionally, the CDS/Employment Supervisor is expected to attend staff meetings at least quarterly that may be scheduled outside of regular hours. Every effort will be made to accommodate the scheduling needs of the counselor when planning these meetings.

Essential Duties and Responsibilities:

• Schedule staff to provide appropriate coverage and maximize effectiveness in addressing participants' needs in a variety of aspects of their lives (personal care, leisure activities, and community integration). The staff schedule should coincide with the schedules of the Meaningful Day Community Development community activities.
• Building and maintaining a working partnership with the program volunteer training, community business for employment, and community sites.
• Ensure activity calendars are established monthly for participants(group and individual)
• Oversee the Employment Services Program's daily operations.
• Work with Employment team members to build relationships with employers in the community; secure employment for identified participants and the delivery of ongoing and/or following support.
• Oversight of Community Development Services program incident reporting process and all incident-related documentation.
• Review attendance daily for final approval via the Meaningful Day Program Manager.
• Ensuring staff follow company policy on natural disaster procedures. ensuring monthly disaster drills are completed in the community.
• Work in conjunction with the Meaningful Day Program Manager and support team to develop goals, Behavior Plans, and Community Development Services Plans.
• Create annual or revised PCPs and BP in iCare with established goals. outcomes and strategies.
• Ensure that the staff are trained on the annual PCP, the implementation of goals, and data collection methods.
• Ensure PCP data is maintained by reviewing the care tracker to ensure documentation.
• Participate in annual Pre PCP and PCP meetings.
• Develop goals with input from participants, family, and Direct Support Staff.
• Ensure that all monitoring and training documents are provided to the CCS upon request.
• Ensure the Behavior Plan Program is correctly implemented by observing and by reviewing data and incident reports.
• Ensure that all individuals are maximally involved in community activities, with plans available to support individuals with special and/or behavior concerns.
• Ensure that all appropriate medical and health maintenance procedures/services are provided utilizing the guidance and direction of the Delegating nurse.
• Ensure that necessary documentation is maintained as follows: Care tracker, Medical Records, and individual files in ICARE.
• Ensure the implementation of all relevant state, local, and federal regulations as indicated by DDA and OHCQ.
• Oversee completion of bi-weekly payroll.
• Meet with Direct Support Staff at least once a month to discuss individual issues/concerns.
• Represent EBED at community and agency-sponsored events as designed.
• Ensure fundamental rights, dignity, and self-respect of all participants.
• Meet with the Meaningful Day Program Manager as scheduled and inform them of relevant issues (agenda/minutes).
• Participate in the Admission/Review/Discharge process and follow-up.
• Performs other job-related duties as assigned.

Experience and Qualifications:

• Bachelor's degree in health or human services preferred or 4 years or related work experience.

• Must complete a successful drug test and criminal background check.
• Ability to work independently using excellent judgment, demonstrated clinical skills, and ability to handle sensitive medical matters in an appropriate manner.
• Must have a current PPD.
• Must have all state-required training.
• Expected to travel to sponsored residential sites and other outside community settings.
