Job summary

Intermountain Centers For Human Development, Inc. in Arizona is seeking a Community Counselor to provide strength-based interventions that build self-regulation and coping skills in community, school, or home settings.

You will collaborate with families, coordinate care, and may facilitate group counseling sessions while participating in care planning and crisis response as needed. This role supports an integrated behavioral health approach across services.

Qualifications

  • Associates degree preferred; high school diploma or GED required.
  • Minimum 4 years of direct member care experience in Behavioral Health.

Responsibilities

  • Provides direct family centered clinical services and care coordination to meet member needs.
  • Prepares written diagnostic assessments from multiple perspectives (family, psychology, social, medical, etc.).
  • Facilitates Child and Family Team meetings and coordinates with providers and professionals.
  • Interprets and follows treatment plans when appropriate.
  • Responds to crisis situations during the workday as needed.
  • Documents case activities for clinical and billing purposes per agency and AHCCCS standards.
  • Works with Clinical Supervisor to increase effectiveness of therapeutic intervention.
  • Performs other duties as assigned as they relate to the position.

GENERAL SUMMARY

The Community Counselor provides strength-based interventions to build self- regulation skills and develops positive coping strategies for anger, impulse control, depression, anxiety and stress, conflict resolution and general self management. Services will be provided to member and/or family in a community, educational, or home-based setting. Care coordination and Child and Family Team (CFT) facilitation skills will also be required and provided as needed based on the needs of the member. Community counselor may also assist in the facilitation of group counseling sessions on a rotating basis.

JOB RESPONSIBILITIES
  • Provides direct family centered clinical services to members as well as care coordination to meet the needs of the member and employee productivity standards. Services include individual, family, and group skills building interventions and counseling services to address a variety of complex issues such as behavioral challenges, parenting skills, family systems issues, anger management, and substance abuse interventions.
  • Prepares written diagnostic assessments, assessing service recipients from multiple perspectives, including family functioning, psychological/emotional, social, academic, medical, behavioral, substance abuse, skills, strengths, barriers to self-sufficiency, treatment history, and others, and utilizing various assessment instruments.
  • Facilitates Child and Family Team (CFT) meetings. Collaborates and coordinates with other service providers, system partners, and professionals, such as attorneys, school personnel, social workers, and probation officers.
  • Interpretand followtreatmentplan from otherproviderswhenclinicallyappropriate.
  • Respondstocrisissituationsduring theworkdayasneeded.
  • Documentscaseactivitiesforboth clinicaland billingpurposesin accordancewithagencyand AHCCCSstandards.
  • Workswith ClinicalSupervisortoincreaseeffectivenessoftherapeuticintervention.
  • Performsotherdutiesasassignedornecessaryasthey relate tothegeneralnatureoftheposition.
QUALIFICATIONS
  • Education – Associatesdegreepreferred,high schooldiplomaorGED required.
  • Experience – Minimum4yearsofdirectmembercareexperienceinBehavioralHealth.
REGULATORY
  • Minimum 18 years of age.
  • Arizona Level One Fingerprint Clearance Card (must possess upon hire and maintain throughout employment).
  • CPR, First Aid & AED certification, if required (must possess upon hire and maintain throughout employment).
  • Valid Driver’s License, 39-month Motor Vehicle Report, proof of vehicle registration and liability insurance that meet company insurance requirements, if required.
  • Negative TB test result, if required (Employer provides).
