Job summary

Creating a Legacy, Inc. in Indio, CA is seeking a compassionate Case Manager to help adults with developmental disabilities achieve greater independence and quality of life.

You will visit clients in their homes and communities, coordinate services, and develop individualized plans. This full-time, mobile role involves traveling to client locations, documenting progress, and collaborating with families, providers, and professionals to promote autonomy and dignity.

Qualifications

• Bachelor's degree in a related field or equivalent combination of education and experience.
• 2+ years of Case Management experience.
• 1+ year of supervisory experience preferred.
• Experience coordinating services, resources, or care plans is highly valued.
• Ability to travel to client locations and work in the field.

Responsibilities

• Coordinate individualized services and resources based on each client's needs and goals.
• Conduct face-to-face home and community visits.
• Complete and update client assessments and service plans within established timelines.
• Monitor client progress and review plans, reports, and supporting documentation.
• Work collaboratively with clients, families, healthcare professionals, social workers, and service providers.
• Identify barriers and develop practical solutions to support positive outcomes.
• Maintain accurate, timely, and confidential case records.
• Help clients access appropriate community resources and services.
• Promote client choice, independence, dignity, and self-sufficiency.
