Job summary

The Department for Persons with Disabilities in Dover, NJ, is seeking individuals to provide non-clinical support services to consumers in need. You will be responsible for assessing clients, educating them on chronic disease management, and coordinating care plans.

Qualifications

• Bachelor’s Degree or Associate’s Degree with 3-5 years’ work experience preferred.
• Fluency in English and Spanish is preferred.
• Must possess a valid NJ driver's license and vehicle access.

Responsibilities

• Assess clients with community living needs related to health and well-being.
• Educate clients on managing chronic diseases.
• Identify interventions to ensure safe community living.
• Prepare monthly and quarterly reports.
• Participate in strategic planning and quality improvement activities.
