Job summary

Chimes in Kennett Square, PA is seeking a Case Manager to deliver community-based behavioral health services. You will assess needs, monitor resources, and help clients live safely in the community.

The role requires a relevant degree or RN licensure, a strong case management background, and the ability to handle a heavy caseload with crisis support. Full-time schedule is Mon-Fri, 9:00 am–5:00 pm.

Qualifications

• A bachelor’s degree in sociology, social work, psychology, or related field is required.
• RN licensure is an acceptable alternative.
• May substitute education/experience at Personnel Officer’s discretion.

Responsibilities

• Carry a caseload of up to 30 clients.
• Perform case management duties for consumers in Behavioral Health Services.
• Develop strength-based service plans with measurable outcomes.
• Assist consumers with housing, healthcare, employment and social supports.
• Provide crisis assessment and intervention when necessary.
• Maintain coordination among consumer’s service providers and supports.
• Submit accurate documentation for re-authorizations and payroll/billing.
• Be available for an on-call rotation 24/7.
