Job summary

FamilyTIES of SC, LLC seeks a part-time CLTC Case Manager to coordinate care for participants in Columbia, SC. You will conduct in-person and phone visits at initiation, monthly, quarterly, and annually, building partnerships with providers and SCDHHS to ensure safe, effective and patient-centered care.

Qualifications

• Bachelor degree in a health or human services field.
• At least two years of case management experience.
• Certified case manager is a plus.

Responsibilities

• Coordinate and provide CLTC case management to participants.
• Perform face-to-face and phone visits at scheduled intervals.
• Develop collaborative relationships with providers and SCDHHS.
• Ensure documentation meets requirements and regulations.
• Assess, plan, implement, monitor, and evaluate client needs.
• Advise clients to promote wellness and autonomy.
• Maintain client records and produce reports.
