FamilyTIES of SC, LLC is seeking a part-time CLTC Case Manager to coordinate care for participants in the SC CLTC program. The role involves direct client contact, face-to-face and by phone, during initial, monthly, quarterly, and annual visits.

You will work with public and private providers and SCDHHS to ensure appropriate, compliant care and documentation. Strong communication, organization, and advocacy are essential.

Qualifikationen

• Bachelor degree in health or human services required.
• Minimum two years' experience providing case management services.
• Certified case manager is a plus.

Aufgaben

• Coordinate and provide safe, timely, effective, equitable, client-centered care.
• Handle case assignments, draft service plans, review progress and close cases.
• Help clients achieve wellness and autonomy.
• Facilitate care coordination and information sharing across providers.
• Advocate for patients regarding clinical status and treatment options.
• Collaborate with medical team throughout case management process.
• Record case information, complete forms and generate statistics.
• Promote quality and cost-effective interventions and outcomes.
• Address motivational and psychosocial issues.
• Adhere to professional standards and regulations.
