Job summary

St. Vincent de Paul Dayton is seeking a Case Manager to coordinate social services for women and families in the Kettering Commons program. You will work with individuals and families to support parental skills, youth wellness, and educational needs, collaborating with schools and community resources.

The role emphasizes person-centered approaches, ongoing assessments, and up-to-date documentation, with after-hours on-call duties and a commitment to serving diverse populations.

Qualifications

• High School diploma or equivalent.
• 1-3 years of experience with substance use disorder clients.
• 1-3 years of experience with sexual abuse and domestic violence.
• 1-3 years of experience with culturally diverse populations.
• Hold a valid Ohio driver’s license in good standing.
• Microsoft Office proficiency required.
• CCDC or CCDC II certification preferred.

Responsibilities

• Assist clients in obtaining social service supports and linkage to outside providers.
• Develop and maintain goals for individuals and update every six months.
• Coordinate with schools and family systems to support youth needs and interventions.
• Maintain accurate and timely records and progress notes.
• Serve as liaison with community resources and other agencies.
• Assist families with housing readiness plans and independent housing goals.
