Job summary

Juneau County Human Services Department is seeking a Case Manager to provide services to children and adults with varying abilities. The role focuses on assessments, crisis intervention, case planning, advocacy, and connection to community resources.

The position will be assigned to a designated Division within the Human Services Department and requires collaboration with schools, courts, healthcare providers, and other community resources.

Qualifications

• Bachelor’s degree in Human Services or related field.
• Must successfully pass caregiver and criminal background checks.
• Substance Abuse Professional certification may be required in some assignments.

Responsibilities

• Provides assessment, treatment planning, and case management services per program rules.
• Engages service recipients and families to develop goal-oriented service plans.
• Coordinates access to services and monitors progress to ensure timely delivery.
• Maintains complete case records in the agency's systems.
• Educates clients about available resources and coordinates with community partners.
