Job summary

Merakey is seeking a Case Manager for the Children\'s Behavioral Health Program serving clients in Kent County, Delaware. The role involves coordinating resources, documenting case activity, and supporting discharge planning with family participation.

The position requires a Bachelor\'s degree, at least 1 year with children/adolescents, and a valid driver\'s license. Some evenings, weekends, and on-call coverage may be required.

Qualifications

• Bachelor's degree required.
• At least 1 year of experience working with children/adolescents.
• Valid Driver's License and clean driving record.
• Able to work Monday-Friday with an on-call rotation.

Responsibilities

• Coordinate resources and information for client needs.
• Document all coordination of resources.
• Coordinate referrals for discharge planning with family input.
• Monitor and update client charts and incident reports within 24 hours as needed.
• Assess and document safety.
• Complete required case documentation per Merakey and licensing standards.
• Deliver case management sessions primarily in the community (homes, schools); may be in an office or community setting; some evenings/weekends and on-call coverage may be required.
