Merakey is seeking a Case Manager for the Children's Behavioral Health Program serving clients in Kent County, Delaware. Qualifications

• Bachelor's degree required.
• Minimum 1 year experience with children/adolescents.
• Valid driver’s license with clean driving record.
• Ability to work Monday-Friday with on-call rotation.

Responsibilities

• Coordinate resources and information for client needs.
• Document all coordination of resources accurately.
• Coordinate referrals for discharge planning with family participation.
• Maintain up-to-date charts and incident reporting within 24 hours.
• Monitor safety and compliance across documentation.
• Deliver case sessions primarily in the community (homes, schools, etc.).
• Occasional evenings or weekends; on-call coverage may be required.
