Job summary

Wheeler Clinic in Plainville, CT seeks an Extended Day Treatment Program Case Manager to provide case management services for children and families. You will foster positive relationships with clients, families, and outside key stakeholders.

This full-time role requires a BA/BS degree, a Connecticut Driver's License, and a Public Passenger “A” endorsement within 90 days. You will coordinate referrals, support counseling, maintain records and participate in team rounds, with a strong commitment

Qualifications

• BA/BS degree in related field is required.
• Valid Connecticut Driver's License.
• Must possess or obtain and maintain a Public Passenger “A” endorsement within 90 days of employment.
• Safely operates vehicle in the community, based on program needs.

Responsibilities

• Provides coordination for referrals and linkages with community referral sources.
• Co-Facilitates group and/or individual counseling with appropriate supervision.
• Maintains a structured and safe clinical milieu environment; utilizes a team approach with evidence-based practices.
• Provides direct case management services and develops case management plans specific to each assigned client and family, including interventions to increase treatment completion and improve life functioning.
• Maintains records of case management activities for reporting and tracking and completes documentation for client medical records per timelines and regulations.
• Attends team meetings and staff meetings to coordinate case management and review treatment plans.
• May transport clients to appointments or linkages as appropriate.
• Provides service in the office and/or community settings outside the office as defined by the program.
• Administers standardized screening tools and collects data as defined by program.
• Provides/supports safety planning and crisis intervention; may include after hours on-call responsibilities.
• Transports clients as required.
• Develops knowledge about cultural histories and perspectives relevant to groups served and applies appropriate methodologies.
