Job summary

Qualifications

Responsibilities

• Provide strength-based, solution-focused supportive therapeutic services in the community.
• Deliver client-centered, individual PSR services in the community.
• Assess recipient strengths, resources, and goals within 30 days and plan ongoing treatment.
• Participate in collaborative meetings with Rehabilitation Services staff and other agencies.
• Provide crisis intervention services as needed per policies and service definitions.
• Communicate client status regularly, including mood, medical issues and safety concerns.
• Produce effective clinical documentation of every contact with recipient.
• Assist with 90 Day client reviews.
• Attend trainings and regular clinical supervision with Clinical Manager.
• Return phone calls promptly and follow safety rules.
• Complete all yearly trainings.

Responsibilities

• Provide strength-based, solution-focused supportive therapeutic services in the community.
• Deliver client-centered, individual Psychosocial Skills Rehabilitation (PSR) services in the community.
• Assess each recipient for Strengths, Resources, and Goals within first 30 days of treatment; provide Strength-based planning and treatment on an ongoing basis.
• Participate in collaborative meetings with Rehabilitation Services staff, other agencies or community and natural supports as needed.
• Provide Crisis Intervention services to recipients as needed according to agency policies, procedures, and service definitions.
• Communicate client’s status on a regular basis, including changes in mood or functioning, medical issues and all crisis and health and safety issues.
• Produce effective, clinical documentation of every contact with recipient.
• Assist with the completion of 90 Day client reviews
• Attend all office trainings as well as regular clinical supervision sessions with Clinical Manager
• Return phone calls promptly and follow all safety rules.
• Complete all yearly trainings

Qualifications

• Master’s Degree from an accredited university or college in the field of Counseling, Social Work, Psychology or Sociology.
• Ability to use computer interface to complete documentation for billing or other purposes as needed.
• Excellent communication and documentation skills.
• Ability to work collaboratively with a diverse team.
