Job summary

Better Together ND in Bismarck, ND is seeking a dedicated Care Coordinator to support participants across recovery, housing, and employment services. You will partner with local agencies and act as a primary advocate to help clients achieve and sustain overall well-being.

The role requires a Bachelor’s degree or four years of direct service experience, plus a valid driver’s license and willingness to complete state training after hire. In-person work in the Bismarck area is expected.

Qualifications

• Bachelor's degree in a related field or four years of direct human services experience.
• Background check and valid driver's license required.
• Commitment to complete state-provided care coordination training after hiring.

Responsibilities

• Direct Engagement: conduct regular weekly meetings with participants.
• Care Planning: create client-centered care plans focused on goals.
• Inter-Agency Collaboration: coordinate with local agencies for housing, employment, and recovery.
• Documentation: keep timely, accurate records of all interactions.
