Job summary

The Care Team Hospice is seeking a driven, relationship-driven Community Care Liaison (CCL) to expand our presence in the community in Crown Point, Indiana. This role focuses on building connections with physicians, hospitals, and referral sources to increase awareness and utilization of hospice services.

You will educate providers, patients, and families on hospice value and guide them toward compassionate, high-quality care while working with a collaborative team focused on growth and purpose.

Qualifications

• Experience in building referral networks with physicians, hospitals and care providers.
• Ability to explain hospice value to patients, families, and providers.
• Strong communication and relationship management skills.

Responsibilities

• Grow census through partnerships with referral sources and the community.
• Educate providers, patients, and families on hospice benefits and services.
• Serve as the face of hospice services in the community and nurture relationships with key partners.
• Collaborate with internal teams to ensure high-quality patient experience and timely referrals.

As a Community Care Liaison, you will be the face of our hospice services in the community. You’ll build and strengthen relationships with physicians, hospitals, and other referral sources to increase awareness, trust, and utilization of our services.

Your success will be measured by your ability to grow census through strong partnerships, consistent outreach, and a deep understanding of hospice care. You’ll also serve as a trusted resource—educating providers, patients, and families on the value of hospice and guiding them toward compassionate, high-quality care.
