Job summary

Texas Health and Human Services Commission (HHSC) in Harlingen, TX seeks a Community Care Worker III to support the STAR Kids MDCP and HCBS enrollment. The role handles eligibility, waiver decisions, and transitions from nursing facilities to community care, using TIERS/SASO and LTCOP systems.

The position requires strong communication, time management, and problem-solving skills with the ability to coordinate across MCOs and ME&P staff. Telework options are available as per policy.

Qualifications

• High School Diploma or GED required.
• Prefer experience with case management.
• Prefer experience in time management.
• Prefer experience with complex policy interpretation requiring good judgement.

Responsibilities

• Attend work on a regular and predictable schedule and perform other duties as assigned.
• Develop, coordinate, and explain waiver programs to applicants and members; authorize or deny waiver services.
• Validate eligibility in appropriate systems and register waiver program eligibility in the authorization system.
• Coordinate with MEPD for enrollment and support members transitioning from facilities to the community.
• Assist denied waiver members to file fair hearings and track processes to a decision.
