Job summary

Lutheran Social Services of Wisconsin & Upper Michigan is seeking a Service Facilitator to join the Comprehensive Community Services program in Trempealeau County. The role involves partnering with individuals, families, and providers to assess needs, develop personalized service plans, and coordinate services that promote independence and well-being.

Qualifications

Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field.
Experience working with individuals, families, or vulnerable populations preferred.
Strong communication, organization, and relationship-building skills.
Ability to manage documentation, coordinate multiple services, and work independently.
Valid driver's license, reliable transportation, and ability to travel within the community.

Responsibilities

Conduct functional and needs assessments.
Develop and implement person-centered service plans.
Coordinate services and connect clients with community resources.
Facilitate team meetings and collaborate with families, providers, and support networks.
Maintain timely, accurate documentation and client records.
Advocate for clients using trauma-informed, strengths-based practices.
