Job summary

Lutheran Social Services of Wisconsin & Upper Michigan is hiring a Service Facilitator for the Comprehensive Community Services (CCS) program in Trempealeau County, WI. Qualifications

  • Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field.
  • Experience working with individuals, families, or vulnerable populations preferred.
  • Strong communication, organization, and relationship-building skills.
  • Ability to manage documentation, coordinate multiple services, and work independently.
  • Valid driver's license, reliable transportation, and ability to travel within the community.

Responsibilities

  • Conduct functional and needs assessments.
  • Develop and implement person-centered service plans.
  • Coordinate services and connect clients with community resources.
  • Facilitate team meetings and collaborate with families, providers, and support networks.
  • Maintain timely, accurate documentation and client records.
  • Advocate for clients and support positive outcomes through trauma-informed, strengths-based practices.
