This is an ideal position for a CNA or experienced home care professional who has hands‑on caregiving experience and is ready to take the next step into a coordination and leadership role.
The Community Care Coordinator will oversee care delivery across multiple senior living communities in Cary and Raleigh, ensuring residents receive reliable, compassionate, and personalized care while supporting caregivers, families, and our office team.
Our Community Care program is different from traditional one‑on‑one home care. Caregivers may assist multiple residents throughout the day, making strong organization, communication, time management, and leadership skills essential to success.
The ideal candidate is someone who:
We are looking for someone who is more than just a caregiver. We need someone who can be a leader, advocate, trainer, problem‑solver, and strong communicator.
You should be comfortable walking into a community and asking:
“Are we clients receiving the care they need?”
“Are our caregivers supported and following the care plans?”
“Are families happy with the communication and care?”
“What needs to be addressed before it becomes a bigger problem?”
You will play an important role in making sure our clients receive the quality of care they deserve and that our caregivers feel supported and prepared to provide it.
