Job summary

Palms Medical Group, a not-for-profit FQHC serving North Florida, seeks a Case Manager to coordinate patient referrals and ensure timely, quality care. You will liaise with patients, providers, and specialists, secure authorizations, and identify community resources to address barriers to care.

Qualifications

• Must be organized, a self-starter and detail oriented.
• Ability to work independently and within a team.
• Willingness to multitask in a fast-paced environment.
• Knowledge of local referral sources and ability to mentor coworkers.

Responsibilities

• Provide professional, courteous patient support following PMG policies.
• Manage referral workflows, obtain authorizations, and coordinate with patients, providers, and specialists.
• Identify community resources and help patients overcome language or social barriers to care.
• Follow EHR referral tracking, document reports, and close orders per PMG policy.
• Obtain insurance authorizations and present medical necessity information to insurers.
• Maintain HIPAA compliance and assist with audits and office administration duties.
