Job summary

Houston Food Bank is seeking a Community Resource Navigator to provide state public benefits assistance and referrals across the 18-county service area, traveling to social service agencies, Houston Food Bank sites, and community events, as well as by phone and virtually.

The role handles SNAP, Medicaid, CHIP, and TANF applications, ensures HIPAA compliance, enters data, gathers documentation, and supports partner agencies while delivering excellent customer service.

Qualifications

• High School Diploma or GED with 2 years of related case management
• Bachelor's degree in Social Work or related field may substitute experience
• CHW certification preferred but not required
• Experience in a social service agency is a plus
• Bi-lingual English/Spanish preferred

Responsibilities

• Provide application assistance for state benefit programs via HHSC Self Service Portal or paper applications at Field Sites
• Safeguard client information to ensure HIPAA compliance
• Enter client data and collect/verify documents for eligibility databases
• Assist clients in gathering documentation and advise on additional required materials
• Attend meetings and outreach events; travel to community sites as needed
• Educate clients about program benefits and how to access services
• Submit daily client information to reporting platforms
• Drive a SNAP vehicle for outreach events when required
• Support disaster recovery duties as needed
