Qualifications

• Master's degree in Rehabilitation Counseling or related field.
• CRC certification or enrollment in a qualifying program with graduation prior to hire.
• Knowledge of human services, rehabilitation counseling, psychology or related disciplines.
• Experience coordinating services for people with disabilities.
• Reliable transportation to meet participants in the community.

Responsibilities

• Evaluate referred participants with disabilities for eligibility and services.
• Maintain contact with referral sources and inform eligibility decisions.
• Coordinate employment placement services and assess job suitability.
• Counsel clients to develop individualized rehabilitation plans and goals.
• Prepare documentation and approvals for plans and program authorization.
• Follow up on client progress and adjust plans as needed.
• Work with OOD liaison to maximize caseload and ensure timely approvals.
