ViaQuest, Inc. invites applications for a Community Based/Day Program Therapist in Cincinnati, Ohio. This full-time role supports children, adolescents and adults in home and community settings, delivering assessments and evidence-based therapy within a multidisciplinary team.

Qualifications

• Bachelor's degree in Social Work or related field with one year of therapy experience or a Master's degree in Social Work or Professional Counseling.
• Licensed: LSW/LISW/LISW-S/LPC/LPCC/LPCC-S/LMFT required.
• Experience in therapy and home settings preferred.
• Valid driver's license and reliable transportation; willingness to travel.

Responsibilities

• Perform comprehensive diagnostic assessments.
• Provide patient visits as scheduled.
• Ensure treatment plans are up to date for each client.
• Complete comprehensive and succinct progress notes.
• Communicate and follow up regarding the client's status.
• Collaborate with case management and/or psychiatric services.

Responsibilities

• Performing comprehensive diagnostic assessments.
• Providing patient visits as scheduled.
• Ensuring treatment plans are up to date for each client.
• Completing comprehensive and succinct progress notes.
• Communicating and following up as needed regarding the client's status.
• Working collaboratively with those providing case management and/or psychiatric services.

Requirements

• Bachelor's degree in Social Work or a related field with one year of therapy experience or a Master's degree in Social Work or Professional Counseling.
• Social Work/Professional Counseling License or equivalent: LSW, LISW, LISW-S, LPC, LPCC, LPCC-S, LMFT.
• Experience in therapy and counseling is preferred.
• Experience within a home setting is preferred.
• Valid driver's license and reliable transportation.
• Willingness to travel throughout assigned service area.
