Job summary

TBMHC is seeking a Community-Based Mental Health Therapist to deliver direct clinical services in homes, schools, and community settings. You will conduct intakes, diagnose needs, and provide individual, family, and group therapy with trauma-informed, culturally responsive approaches.

Responsibilities include treatment planning, collaboration with schools and partners, crisis intervention, and ongoing professional development.

Qualifications

  • Bachelor's degree in Social Work, Psychology, Counseling or related field.
  • Ohio licensure: LSW, LPC, MFT (independent licensure preferred).
  • Passion for serving underserved populations and addressing mental health disparities.
  • Strong engagement skills and cultural humility.
  • Commitment to ethical, person-centered care.

  • Provide diagnostic assessments to determine eligibility and treatment needs.
  • Facilitate individual, family, and group therapy using culturally responsive, evidence-based approaches.
  • Deliver crisis intervention, safety planning, and risk assessments when needed.
  • Develop collaborative treatment goals with clients and families.
  • Identify client strengths, needs, and barriers to inform the ISP.
  • Modify treatment plans as needed and communicate progress during treatment team meetings.
  • Coordinate care with community partners, schools, and other service providers.
  • Complete treatment plans, progress notes, and discharge summaries in a timely manner.
  • Maintain productivity expectations and meet all state, federal, and funding requirements.
  • Participate in clinical supervision and ongoing professional development.
  • Connect clients to community resources and ensure follow-up on referrals.
  • Support clients navigating trauma, addiction, grief/loss, family conflict, chronic stress, and other challenges.
  • Provide services where clients feel most comfortable—home, community, school, or telehealth.

Documentation & Compliance

  • Complete treatment plans, progress notes, and discharge summaries in a timely manner.
  • Maintain productivity expectations and meet all state, federal, and funding requirements.
  • Participate in clinical supervision and ongoing professional development.

Community Engagement
