Job summary

TeamBuilders is a community-based behavioral health organization providing housing support services for adults, children, and families. The SHC role focuses on comprehensive community support, monitoring and evaluating services for eligible clients, and carrying a caseload within the home and community.

The position involves CCSS activities such as assessment, care and safety planning, skills development, monthly visits, budgeting, and coordinating with other services to ensure effective housing

Qualifications

• Minimum qualifications include a degree in a human services field and experience with adults with Serious Mental Illness (SMI).
• Alternative combinations include an Associate degree with 2+ years of SMI experience or a high school diploma with 3+ years of SMI experience.
• It may also meet NM CPS or CFS certification requirements.

Responsibilities

• Monitor, track, and evaluate services for eligible clients.
• Carry a caseload as part of the community support team.
• Perform CCSS activities in home and community settings.
• Engage in assessment, care planning, safety planning, and skills development.
• Coordinate with other services and budgets as needed.

Education

Bachelor’s degree in a human services-related field

Associate degree in a human services-related field

High School Diploma or GED

Meets NM minimum CPS or CFS requirements
