Job summary

REM Community Services, a part of the Sevita family, seeks a Registered Nurse to provide nursing services, training, and support for individuals with complex medical and behavioral needs in a community setting.

You will assess medical needs, develop care plans, administer medications, and collaborate with physicians to ensure high-quality care while promoting health and wellness across clients.

Qualifications

• Graduate of an accredited RN program.
• Current state RN License.
• Current CPR/First Aid certification.

Responsibilities

• Conduct medical assessments for individuals referred to the program and collaborate to determine appropriate placement in the community.
• Develop and implement service plans; supervise medical components with physicians.
• Review and revise nursing care plans regularly; maintain current medical records and reports.
• Maintain contact with primary care physicians and specialists; attend doctor visits as directed.
• Promote health and wellness; ensure nursing actions are in place for prevention.
• Administer medications per established guidelines and document administration records.
• Oversee management of high-risk medical conditions and private duty nursing services.
• Develop training flow charts and provide medical skills training to clients, mentors, staff, and families.
• Participate as an IHP/POC team member; evaluate needs and ensure goals are medically indicated.
• Provide consultation on home care issues and operation of durable medical equipment.
• Coordinate household activities and transport participants to planned activities; participate in on-call system as needed.
• May participate in IDT meetings, Human Rights Committee, Health and Safety Committee, and Administration meetings.
• Perform other related duties as required.
