Job summary

Santa Clara Family Health Plan seeks an experienced Community-Based Program Specialist to lead and support ECM, CS, CHW networks across Santa Clara County. You will work to develop, support, and monitor provider relationships and performance, ensuring regulatory compliance and effective service delivery.

The role requires collaboration with multiple internal units, managing data, and driving improvements in care access and outcomes for underserved communities.

Qualifications

• Bachelor’s degree in public health, social welfare or related field, or equivalent experience.
• At least two years of progressive experience in related duties.
• Project management experience and ability to lead or direct work.
• Knowledge of community-based services and health disparities in Santa Clara County.
• Understanding of SDOH and ability to analyze data and write reports.
• Strong communication, prioritization and problem-solving skills.
• Proficiency with Microsoft Office suite and related software.

Responsibilities

• Develop and maintain knowledge of community-based services and ECM/CS/CHW networks.
• Identify needs to support ECM/CS/CHW delivery and capacity building.
• Support delivery processes per DHCS definitions and reporting requirements.
• Oversee the provider network and manage relationships with vendors and providers.
• Communicate with providers to identify issues and develop improvement opportunities.
• Collaborate with internal units to ensure payments align with vendor terms.
• Schedule site visits, provide training, and develop guidance materials.
• Monitor compliance with agreements, work scope, and performance metrics.
