Job summary

Rainbow Community Care is seeking a Community-Based Palliative Care Nurse Practitioner to support patients with serious illnesses across home and facility settings. You will perform in-home evaluations, manage complex symptoms, and lead goals-of-care conversations with families and caregivers.

Join a mission-driven team dedicated to improving quality of life. A Wisconsin NP license, prescriptive authority, DEA registration, and a Master’s in Nursing are required, with CHPNP preferred.

Qualifications

• Active, unrestricted Wisconsin NP license with prescriptive authority (APNP).
• Certification from an approved certifying body for advanced practice nursing (e.g., ANCC, AANP).
• CHPNP® – Certified Hospice and Palliative Nurse Practitioner; preferred
• Current DEA registration.

Responsibilities

• Provide in-home and facility-based care to patients with serious illnesses, performing assessments and managing symptoms.
• Engage in goals-of-care conversations and advance care planning with patients and families.
• Collaborate with care teams to coordinate services and ensure continuity of care.
• Order and interpret labs and diagnostic tests, document findings in the EMR.
