Job summary

InHome Connect is seeking a Nurse Practitioner - Palliative Care to join our community-based team dedicated to compassionate end-of-life care. You will collaborate with physicians, educate patients and families, and help develop individualized care plans in home settings.

Ideal candidates have RN and APN licenses, strong pharmacology knowledge, and at least two years of APN experience in hospice or home health. We offer meaningful work in a supportive, compliant environment.

Qualifications

  • Graduate of a Baccalaureate School of Nursing.
  • Current RN license and current APN license in the state.
  • Current Driver's License, reliable transportation, and provide ongoing auto liability insurance.
  • Two years of APN clinical experience; hospice or home health preferred.
  • Comprehensive knowledge of pharmacology, palliative care, and end-of-life care.

Responsibilities

  • Consult with and educate the patient/family regarding disease process, self-care techniques, end-of-life care
  • Consult with and assist in the IDG's development of patient/family plan of care to assure quality care is provided
  • Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician
  • Inform physicians and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care
  • Timely submission of all documentation
  • Performance of other duties as required
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures

What You Must Have:
  • Graduate of a Baccalaureate School of Nursing
  • Current State license as a Registered Nurse and as an Advanced Practice Nurse (APN)
  • Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
  • Two years of experience as an APN in a clinical care setting, hospice or home health preferred
  • Comprehensive knowledge of pharmacology, palliative care, and end-of-life
