Qualifications

• Bachelor’s degree in Human Services or related field required.
• Minimum of 1 year of experience in community mental health or related field preferred.

Responsibilities

• Provide community-based case management services to individuals with severe and persistent mental illness.
• Support individuals in achieving independence, stability, and community integration.
• Coordinate person-centered services to connect individuals with medical, social, educational, and community resources.
• Assist individuals in identifying and developing natural supports within their community.
• Advocate for consumer choice and individualized care.
• Collaborate with clinical teams and community partners to ensure comprehensive support and care coordination.
