Qualifications

• Minimum 3 years caregiving experience.
• Valid driver's license and reliable transportation.
• Current certifications in Adult First Aid & CPR/AED, DDA, and MANDT.
• Ability to pass a state background check.

Responsibilities

• Provide community-based support focused on skill-building, engagement and independence.
• Assist with daily living tasks such as communication, hygiene, meal prep, organization and transportation.
• Help individuals access recreational, social, and volunteer opportunities.
• Set personal goals and track progress regularly.
• Build positive, respectful relationships and advocate for inclusion.
• Maintain accurate documentation and follow agency standards.
• Follow safety protocols in community and home settings.
• Provide transportation to appointments, meetings and events.
• Encourage skill development and community integration.
• Attend trainings and obtain required certifications as needed.
