Qualifications

• Must be fully licensed and able to perform without supervision.

Responsibilities

• Conduct in-person wellness assessments for older adults.
• Represent Total Life in the community as a local point of contact.
• Collaborate with the virtual therapy team for smooth care transitions.
• Maintain accurate and timely clinical documentation.
• Participate in team meetings to improve community-based programs.

Skills

Strong background in evidence-based modalities

Strong interpersonal and communication skills

Experience working with older adults or geriatric mental health

Bilingual in English/Spanish

Education

Active LCSW, LMFT, LPC, or LMHC license in Texas
