Requirements

Position Details

The Case Manager assesses, plans, and implements recovery treatment and support plans for adults and youth with mental health diagnoses and/or on the autism spectrum. They provide intensive therapeutic services in various settings, including community living arrangements, schools, and community settings, utilizing a person-centered approach in collaboration with the treatment team.
