The Aging and People with Disabilities (APD) program is hiring multiple Community-Based Care Kitchen Surveyor's (Client Care Surveyor) to join our Community-Based Care (CBC) team in Tigard, Oregon.

Salary Information: Employees who are not yet PERS members (AA) will also get the 6.95% pay increase and start the 6% contribution when they join PERS (AP).

Client Care Surveyor (AA): $5,575 – $8,550

Client Care Surveyor (AP): $5,965 – $9,143

Summary of Duties

As a Community-Based Care Kitchen Surveyor , you will:

  • Conduct on-site annual kitchen inspections according to state protocols, guidelines, and time frames.

  • Review previous kitchen inspection reports for compliance history. Observe infection control practices and food preparation according to Oregon Food Sanitation rules.

  • Interview Person In Charge, facility administration, and kitchen staff.

  • Review kitchen staff training records, policy, and procedures.

  • Evaluate kitchens according to OARs, Oregon Food Sanitation, to determine findings of potential non-compliance.

  • Write report of findings, using specified computer software program.

Minimum Qualifications

  • A valid driver’s license and acceptable driving record are required for this position.

  • Three years of experience in areas related to long-term care dietitian and or food service manager, AND a bachelor’s degree in health science, dietetics, and/or food management.

  • Rules/Regulations: Basic knowledge of Oregon State laws, rules, and Oregon Food Sanitation rules.

  • Critical Thinking, investigation, and decision making: Skill in exercising independent judgment and decision-making in the application of laws, rules, policies, or procedures.

  • Teamwork, self-awareness, and growth mindset: Demonstrated ability to work successfully both independently and cohesively with a team. Ability to adapt to different team dynamics and accept feedback.

  • Conflict Resolution, Emotional Intelligence, and Professionalism. Ability to maintain professionalism under tense or hostile conditions.

  • Computer Skills: Experience using a variety of Word processing software, spreadsheet software, email management, data entry, video conferencing, instant messaging, and managing a laptop.

  • Technical skills: Solid writing skills using the conventions of report writing and skill in communicating verbally with a variety of medical professionals, care provider management and staff, resident relatives, and personnel or other agencies.

Working Conditions

  • The majority of your work will be conducted in the office and community.

  • Trail service and training will take place in Tigard, Oregon for 9 months.

  • Travel frequently, often overnight, to counties throughout Oregon up to two weeks out of the month.

  • Work daily within busy and varying long-term care healthcare facility settings.

  • Lift and carry up to 20 pounds throughout each day.

  • Irregular work schedules and frequent schedule changes.

  • You may interact with a wide range of people including some who have experienced trauma and exhibit challenges in controlling their emotions.

  • Walking into stressful situations that may require quick decision-making skills to ensure safety of those being serviced and own physical safety. This may also be trauma inducing.

  • Your office work location will be Tigard.

  • You will be a mandatory reporter of abuse and neglect of children, older adults, and other adults with disabilities.

  • The Oregon Department of Human Services does not offer visa sponsorship.
